Dr. Janiga is contracted as a provider with most major health insurance plans. If you have any questions about your coverage, including the need for a referral from your primary physician, please check with your health insurance plan prior to your appointment with him. If you have been referred by your primary care clinic, it is your responsibility to have the proper authorization for your appointment. Prolotherapy is not covered under most insurance plans. Patients receiving Prolotherapy should expect to pay at the time of service to obtain the discounted price. |
